AHC - Antioch Health Center Directions: Take 4 East, then Exit at the A Street Exit towards Lonetree Way; Turn right on Lone tree Way then take a U turn at Davison drive, enter the first driveway on your right and park in 1st lot. Entrance is to the left of building as you see it from parking lot, 1st door on your left. If this door is locked, walk to end of hall, and the last door on left should be open.
BHC - Brentwood Health Center Directions: From the hospital take Hwy 4 East past Pittsburg and Antioch. Continue onto Alternate Hwy 4 and get off on Sand Creek Road, you will go on this for about 10 minutes, passing about 3 stop lights. After you go over the railroad tracks, look to your right and you will see a big, white, modern-looking complex. Take a left into this complex and drive to the back, then take a right. Look for 171 which is labeled Brentwood Health Center.
CHC - Concord Health Center Directions: Take Highway 4 East to 680 South. Exit at Willow Pass Rd. Take a Left off the exit and continue on to 3052 Willow Pass Road. The clinic is in Suite 103.
MHC - Martinez Health Center Directions: Wellness Center - behind the ER
PHC - Pittsburg Health Center Directions: Take Highway 4 East. Exit at Loveridge Ave., turn Right on Loveridge (towards Los Medanos College), turn Left at the second light onto East Leland Rd, then Right into the first driveway. Enter at the main entrance, take the elevator to the 4th floor, go through the door to the right of registration, and turn right. Pediatrics clinic is at the end of the hall.
WCHC - West County Health Center Directions: From Route 4 West, take I-80 W towards San Francisco. Take exit 18 for San Pablo Dam Rd, and turn right onto San Pablo Dam Rd. After 0.3 mi, turn right onto San Pablo Ave. The health center is on the opposite side, so make a U-turn at Pinewood Terrace. The address is: 13601 San Pablo Ave, San Pablo, CA 94806.
